I, Li Grosjean, at the practice Li KINESIOLOGIE, Rue de la Morâche 14, 1260 Nyon, adhere to the following rules in accordance with the data protection law and related regulations.

The purpose of this statement is to inform you, as my client, about how we process your personal and sensitive data collected at my practice.

Only data related to therapeutic treatment or relevant to informing you about potential offers are processed. This includes exclusively data recorded with your consent, data you provide, or data available in public directories (telephone directory, etc.). This may include general personal information (name, address, phone number, etc.), health information/progress of your condition, previously diagnosed conditions, and any other data you provide related to the treatment (patient record).

These data are collected, recorded, and processed exclusively in your patient file or necessary administrative records, particularly in the application for billing according to tariff 590. In these storage locations, all processing steps performed on your data, including responsible persons, can be traced.

Your personal data are only transmitted to individuals or sectors of my practice that need them to fulfill their contractual and legal obligations. These individuals are informed of the applicable data protection rules and are required to comply with them.

Your data or parts of there are made accessible to other persons or institutions (insurers, etc.) only with your explicit consent. Judicial orders or the enforcement of legitimate rights by the practice constitute exceptions.

If your data are collected and processed physically, they are stored in a locked room or cabinet inaccessible to unauthorized third parties. Data collected electronically are securely stored (firewall, password, etc.).

Unless contrary cantonal or other legal provisions apply, the data I collected will be deleted twenty years after your last consultation at my practice.

Written communication between my practice and you is conducted via email, SMS, or WhatsApp. The practice uses the communication method you use/choose as a client. You are aware of potential risks related to data exchange, especially sensitive data (possibility of unauthorized third-party access in the case of unsecured communication channels as mentioned above) and you agree to this. Administrative requests such as appointment rescheduling, invoice copies, medical reports, “Release from Professional Confidentiality” forms, are done by email, either encrypted using a specific program or unencrypted with your written consent.

As a current or former client, you can request at any time a copy of all data we/I have collected about you in a common electronic format. Only physically available data will be scanned for this purpose and sent to you as a PDF or paper document. Generally, data will be provided to you free of charge and within a maximum of 30 days.

The person responsible for all questions related to the processing of your personal data and the exercise of your rights is the therapist responsible for your file.
